BBUUC Nursery Registration 2019-2020

Hello!
We are so glad to have you and your little ones with us at BBUUC. Please fill out this form for children under 4 who will be in the nursery during services and special events so we can best serve your family's needs. Please fill out one form per child.
All of our paid nursery staff are highly qualified and background checked. Childcare is available for children up to age 4 during regular Sunday services 9:45 AM - 11:45 AM and during most adult classes and special events if requested. Older children are welcome in the nursery during adult classes and special events.
Please provide, clearly marked with names, diapers and bottled formula or breastmilk for your child, or feel free to nurse your child wherever you feel comfortable. If you prefer, please contact the DLRE to arrange a private nursing space. 
We try to avoid common allergens such as peanuts. Please indicate any allergies, restrictions, or sensitivities on this form and we will do our best to accommodate. You are welcome to send snacks with your child. If you do choose to send snacks, please be mindful of common allergens. If the situation arises where we have a child or children with life-threatening food allergies, we will designate the nursery an allergen-free zone for those allergens.
Children of all ages are always welcome to remain in the sanctuary or the wiggle room with their families if they so choose. It is an honor to witness the spirit of life manifesting in our children, and we welcome them with open arms - giggles, wiggles, and all.

Are there any circumstances that you would like childcare staff to come find you during services/classes? Outside of emergencies or medical concerns - parents will always be immediately notified in these instances.

If my child needs emergency medical care and I am not present to arrange for medical care, I grant my permission to the RE volunteer or other designated responsible adults of the Buckman Bridge Unitarian Universalist Church to arrange for appropriate care
